• Amusement Device License (Board approval required)
• Billiards License  ($275 non-refundable application fee)
• Business License
• Contractor License
• Crime Free Rental Housing
• Massage Therapy License Application (processed by the Police Department)
• Massage Establishment License ($275 non-refundable application fee)
• Pet Shop Operator's License
• Tobacco Sale License ($275 non-refundable application fee)
• Vending Machine License ($50 per machine per year fee)

Licenses processed and issued by other village departments:

• Liquor Licenses - Issued by the Mayor's Office: 708-403-6186
• Raffle License - Issued by the Clerk's Office: 708-403-6150
• Video Gaming License Application - Issued by the Mayor's Office: 708-403-6186

*Download application - type in information - save file - submit through the permit portal. Hand-written applications will no longer be accepted.
